PDD and a 2nd PPD 10 to 15 days after 1rst
If PDD is positive, then chest x-ray is needed

2 references w/ telephone numbers
Certificate
State I.D. & S.S. Card
Hepititis B consent/declination
M.D. has to check off if the employee can work with or without limitations.
Documents needed for R.N. are as follows: License Physical (same as above)

Infection control training certificate (OSHA)
